What does a full-body skin cancer screening include?
At Memorial, Liv, and Akdeniz, screening starts with dermatologist-led total body examination, digital dermoscopy of suspicious lesions, and full-body photography with AI-assisted mole mapping (FotoFinder/MoleMax). High-risk patients (fair skin, 50+ nevi, family melanoma, prior skin cancer) repeat every 6-12 months. Suspicious lesions get excisional biopsy the same week.
What is the treatment plan for stage I-II melanoma?
Wide local excision with 1 cm margins for stage IA, 1-2 cm for IB-II, plus sentinel lymph node biopsy when Breslow thickness exceeds 0.8 mm or ulceration is present. Staging includes PET-CT and brain MRI for stage IIB+. Adjuvant therapy (nivolumab, pembrolizumab) is discussed at multidisciplinary tumor board per NCCN guidelines.
Who qualifies for psoriasis biologic therapy?
Patients with moderate-to-severe plaque psoriasis (PASI >10, BSA >10%, or DLQI >10) failing topicals, phototherapy, and one systemic agent (methotrexate or cyclosporine). Choice between anti-TNF (adalimumab), IL-17 (secukinumab, ixekizumab), or IL-23 (risankizumab, guselkumab) depends on comorbidities, joint involvement, and prior response. TB/hepatitis screening is required before initiation.
When is Mohs surgery preferred over conventional excision?
Mohs micrographic surgery is preferred for high-risk non-melanoma skin cancers on the face, ears, lips, hands, and genitals, recurrent BCC/SCC, aggressive histologic subtypes (morpheaform, infiltrative), and lesions with poorly defined borders. Cure rates reach 99% for primary BCC. Conventional excision with 4-5 mm margins suffices for low-risk trunk and extremity lesions.
What labs and contraception are required for isotretinoin?
Baseline labs include CBC, ALT/AST, fasting lipids, and pregnancy test (women). Repeat liver and lipids at month 1 and 3. Two contraception methods required for women of childbearing potential — started 1 month before, throughout, and 1 month after treatment — due to severe teratogenicity. Pregnancy tests monthly. Mood changes and dry eyes monitored.
How many laser sessions are needed for tattoo removal?
Q-switched and picosecond lasers (PicoSure, PicoWay) typically need 6-12 sessions spaced 6-8 weeks apart. Black ink clears fastest; green, blue, and yellow require more sessions and specific wavelengths. Old, amateur, and superficial tattoos respond better than fresh professional ink. Skin types IV-VI need careful settings to avoid hypopigmentation.
Who is eligible for dupilumab in atopic eczema?
Dupilumab is approved from age 6 months for moderate-to-severe atopic dermatitis inadequately controlled by topical corticosteroids and calcineurin inhibitors. Eligibility usually requires EASI >16 or affected BSA >10% with significant itch and sleep impact. Loading dose followed by every-2-week injections. No routine lab monitoring required; conjunctivitis is the main side effect to watch.
