Frequently asked about Cardiology

Should I choose coronary bypass (CABG) or stenting (PCI) for multi-vessel disease?
The decision follows the SYNTAX score and ESC/EACTS guidelines. Low SYNTAX (≤22) usually favors PCI with drug-eluting stents; intermediate-to-high scores (≥23), left main disease, or diabetes with multi-vessel involvement favor CABG for better long-term survival. A Heart Team MDT at Memorial or Liv reviews your angiogram, LV function and comorbidities before recommending the optimal revascularization strategy.
Am I a candidate for TAVI instead of open aortic valve surgery?
TAVI candidacy is determined by the STS-PROM score, frailty index, anatomy on CT-angiography (annulus size, calcification, access route) and age. STS ≥4% or prohibitive surgical risk patients are clear TAVI candidates; intermediate and low-risk patients over 65-75 are increasingly offered TAVI per 2021 ESC/EACTS guidelines. Memorial Bahçelievler and Akdeniz perform balloon-expandable and self-expanding TAVI under conscious sedation.
How long is cardiac rehabilitation after CABG surgery?
Standard phase-II cardiac rehabilitation lasts 6-12 weeks with 24-36 supervised sessions, starting 2-4 weeks after discharge. Programs include monitored aerobic training, resistance work, risk-factor education and psychological support. Most international patients complete the inpatient phase (5-7 days) in Turkey, then continue phase-II at home with a tailored plan and remote follow-up from the operating cardiac surgeon.
What's the difference between ICD and CRT-D — which do I need?
An ICD prevents sudden cardiac death in patients with LVEF ≤35% and prior arrhythmia or high-risk cardiomyopathy. A CRT-D adds biventricular pacing and is indicated when LVEF ≤35%, NYHA II-IV symptoms and QRS ≥130 ms (especially LBBB morphology). Device choice follows ESC heart-failure guidelines after echocardiography, ECG and a full electrophysiology evaluation by a board-certified arrhythmologist.
What's the success rate of catheter ablation for atrial fibrillation?
Pulmonary vein isolation by radiofrequency or cryoballoon ablation achieves 70-80% freedom from AF at 12 months for paroxysmal AF and 50-60% for persistent AF, with a 3-5% major complication rate. Memorial and Liv use 3D electroanatomical mapping (CARTO/EnSite) and contact-force catheters. Repeat ablation may be required in 20-30% of cases to maintain sinus rhythm.
Do I really need to take statins for life after a stent?
Yes. After PCI with drug-eluting stents, high-intensity statin therapy (atorvastatin 40-80 mg or rosuvastatin 20-40 mg) is recommended indefinitely to stabilize plaque, lower LDL-C below 55 mg/dL and reduce recurrent MI by 25-30% per ESC dyslipidemia guidelines. If LDL goals aren't met, ezetimibe or PCSK9 inhibitors are added. Stopping statins doubles your event risk within 12 months.
How often should I have echocardiography after heart surgery or a stent?
After CABG or valve surgery, transthoracic echocardiography is performed before discharge, at 1, 6 and 12 months, then annually. After PCI, echo is repeated at 6-12 months and yearly if LV function is normal; sooner if symptoms recur. Patients with prosthetic valves, reduced LVEF, or heart failure follow a 3-6 month schedule.
