Tumor board decision — what is in it?
A written treatment plan from 5 specialists (surgeon, medical oncologist, radiation oncologist, radiologist, pathologist) within 72 hours. Includes staging confirmation, recommended sequence (surgery first vs. neoadjuvant chemo), specific drug regimens, and expected timeline.
Immunotherapy — am I eligible?
Depends on tumor type, PD-L1 status, MSI status, and prior treatments. Most lung, melanoma, kidney, bladder and many GI cancers have approved immunotherapy options. We review your biopsy and run PD-L1 IHC if not already done.
Proton therapy — when is it preferred over IMRT?
Pediatric cancers, base-of-skull tumors, ocular melanoma, and selected re-irradiations. Reduces dose to surrounding healthy tissue. Decision made by the radiation oncology team based on dosimetry simulation.

Am I eligible for CAR-T cell therapy and is it available in Turkey?
CAR-T (axicabtagene, tisagenlecleucel, brexucabtagene) is indicated for relapsed/refractory DLBCL after 2 lines, multiple myeloma after 4 lines, and B-ALL up to age 25. Several Istanbul and Ankara JCI centers are accredited apheresis and infusion sites, with cells manufactured abroad and total turnaround of 4-6 weeks. CRS and ICANS are managed in dedicated cellular therapy units; complete response rates reach 50-80% depending on disease.
When is liquid biopsy (ctDNA) ordered and what does it add to tissue biopsy?
Liquid biopsy is used when tissue is insufficient, the tumor is inaccessible, or rapid genotyping is needed at progression, particularly in NSCLC, colorectal and breast cancer. It detects actionable mutations (EGFR, KRAS, BRAF, ESR1) with sensitivity 70-85% and turnaround of 7-10 days versus 14-21 for tissue NGS. ctDNA is also used for minimal residual disease monitoring after curative surgery.
How does NGS-guided targeted therapy change my treatment plan?
Comprehensive NGS (500+ genes) identifies actionable alterations in roughly 40% of solid tumors — KRAS G12C (sotorasib, adagrasib), EGFR exon 19/L858R (osimertinib), ALK/ROS1 (lorlatinib), BRAF V600E (dabrafenib-trametinib, encorafenib), and HER2-low (trastuzumab deruxtecan). Results are reviewed at the molecular tumor board within 10-14 days, and FDA/EMA-approved targeted agents are matched before chemotherapy where possible.
