QUESTIONS

Frequently asked about Bariatric Surgery

What BMI is required for bariatric surgery in Turkey?
International (IFSO/ASMBS) and Turkish guidelines indicate surgery for BMI ≥40, or BMI 35-39.9 with at least one comorbidity (type 2 diabetes, hypertension, OSA, NAFLD, severe joint disease). For Asian-origin patients and patients with uncontrolled type 2 diabetes, the threshold drops to BMI 30-32.5. Memorial and Liv accept patients aged 16-65 after full pre-op workup.
Should I have gastric sleeve or gastric bypass?
Sleeve gastrectomy (LSG) is preferred for BMI 35-50, no severe reflux, and patients who want a simpler one-step operation with no malabsorption — expect 65-70% excess weight loss at 2 years. Gastric bypass (Roux-en-Y) is preferred for BMI >50, severe GERD, or uncontrolled type 2 diabetes — expect 70-80% EWL plus higher diabetes remission. Both are laparoscopic at Memorial and Liv.
How long do I stay in hospital and in Turkey after bariatric surgery?
Hospital stay is 3-4 nights for sleeve and 4-5 nights for bypass. After discharge, you stay an additional 4-6 days at a partner hotel for a follow-up check, leak test (oral methylene blue or contrast study), and dietitian briefing before flying home. Total stay in Turkey is typically 8-11 days.
How much weight will I lose after bariatric surgery?
Average excess weight loss (EWL): sleeve gastrectomy 60-70% at 2 years; gastric bypass 70-80% at 2 years; mini-gastric bypass / OAGB 75-85%. Total body weight loss averages 25-35% depending on starting BMI and adherence. Most rapid loss occurs in months 3-9; weight stabilizes at 18-24 months. Long-term success requires lifelong dietary discipline.
What vitamins and supplements do I need after bariatric surgery?
Lifelong supplementation is mandatory: bariatric multivitamin (2x daily), calcium citrate 1,200-1,500 mg/day, vitamin D3 3,000 IU/day, vitamin B12 (1,000 mcg sublingual daily or IM every 3 months), iron if menstruating or anemic. Annual blood panel (ferritin, B12, vitamin D, PTH, zinc, copper) is mandatory. Memorial provides a written 5-year supplement and follow-up schedule.
What if I regain weight after bariatric surgery?
Significant regain (>15% of nadir weight) affects 20-30% of sleeve patients and 10-20% of bypass patients at 5-10 years. First step is dietitian and behavioral review plus consideration of GLP-1 agonists (semaglutide, tirzepatide). Surgical revision options include sleeve-to-bypass, sleeve-to-SADI-S, or limb-lengthening of an existing bypass. Memorial and Liv perform 80+ revisions per year.
What is the pre-op liver-shrinking diet?
A 2-week very-low-calorie diet (800-1,000 kcal/day, high protein, low carbohydrate) is mandatory before surgery — it shrinks the left lobe of the liver by ~15-20%, making laparoscopic access safer. Memorial dietitians send a personalized meal plan 4 weeks before surgery and check compliance 1 week before travel. Failure to comply may result in surgery being converted to open or postponed.
