How safe is living-donor liver transplantation for the donor?
LDLT donor mortality at high-volume Turkish centers (Memorial, Liv) is 0.2-0.3%, matching global benchmarks. Right-lobe donation (60% of liver volume) regenerates to 90% within 6-8 weeks. Major complications (bile leak, bleeding) occur in 8-10%. Donors undergo 3-week workup: CT volumetry, MRCP, psychological evaluation, cardiac clearance. Türkiye performs 1,500+ LDLTs annually — more per capita than any country.
What MELD score qualifies me for liver transplantation?
MELD ≥15 is the international threshold where transplant survival exceeds medical management. Turkish centers list patients at MELD 12+ given LDLT availability (no waitlist mortality). Exceptions: HCC within Milan criteria gets MELD-exception points (22-28); hepatopulmonary syndrome, refractory ascites, recurrent variceal bleeding also qualify. Pre-transplant assessment includes endoscopy, echocardiogram, CT chest/abdomen, dental clearance.
Can I receive a kidney from a donor with a different blood type (ABO-incompatible)?
Yes. ABO-incompatible kidney transplant (ABOi-KT) is routine at Memorial and Akdeniz using rituximab + plasmapheresis to deplete anti-A/B antibodies pre-op, plus standard tacrolimus-MMF-steroid immunosuppression. 5-year graft survival matches ABO-compatible (85-90%). How does the paired kidney exchange program work?
When your willing donor is incompatible (blood type or crossmatch-positive), Türkiye's paired exchange (çapraz nakil) matches you with another incompatible pair — donor A gives to recipient B, donor B gives to recipient A. Memorial and Akdeniz coordinate 2-way and 3-way swaps. Both surgeries run simultaneously. Outcomes equal compatible LDKT. Reduces wait from years to weeks; legal framework under Türkiye's 2238 transplant law.
What does lifelong immunosuppression after transplant involve?
Standard triple therapy: tacrolimus (target trough 5-8 ng/mL after year 1), mycophenolate (1-2 g/day), tapered prednisolone (some centers steroid-free by month 6). Monthly bloodwork year 1, then quarterly. Risks: infections (CMV, BK virus prophylaxis), skin cancer (annual derm exam), diabetes, hypertension. Drug costs $300-600/month — Turkish pharmacies offer generic tacrolimus at 40-60% of EU prices.
Can I get a liver transplant for hepatocellular carcinoma (HCC)?
Yes — if within Milan criteria (single tumor ≤5 cm, or up to 3 tumors each ≤3 cm, no vascular invasion, no metastases). 5-year survival reaches 70-75%. Extended UCSF criteria (single ≤6.5 cm or 3 tumors total ≤8 cm) accepted at some Turkish centers with downstaging via TACE or Y-90 radioembolization first. What is the minimum age and weight for pediatric liver transplantation?
Memorial and Akdeniz transplant infants from 5 kg (typically 4-6 months old) for biliary atresia, the leading pediatric LT indication post-failed Kasai procedure. Living-donor left lateral segment (segments 2-3) is standard for children under 20 kg. 1-year patient survival exceeds 90%, 5-year ~85%. Türkiye performs 300+ pediatric LTs annually — second worldwide after the US.
