Frequently asked about Ophthalmology

When should I have cataract surgery — at what symptom stage?
Modern cataract surgery is recommended when lens opacity meaningfully impairs your daily activities (driving at night, reading, glare) — not by waiting for the cataract to ripen. Best-corrected visual acuity below 0.5 (20/40) is a typical threshold, but symptoms matter more than the chart. Memorial and Akdeniz ophthalmology perform femtosecond-laser-assisted phacoemulsification with topical anesthesia and same-day discharge.
Should I choose a multifocal or monofocal IOL for cataract surgery?
Monofocal IOLs provide excellent distance vision but require reading glasses (cheapest, fewest visual side effects). Multifocal/trifocal IOLs (PanOptix, AT LISA tri, Synergy) and EDOF lenses (Vivity, Symfony) restore intermediate and near vision in 80-90% of patients, with mild halos/glare in 10-15%. Choice depends on lifestyle, pupil size, astigmatism and ocular comorbidities — assessed pre-op with biometry and OCT.
Am I eligible for LASIK or SMILE laser refractive surgery?
Eligibility requires age ≥18, stable refraction for 12 months, myopia up to -10 D (LASIK) or -10 D (SMILE), astigmatism up to 5 D, corneal thickness ≥480-500 µm and no keratoconus on topography/tomography (Pentacam). SMILE is minimally invasive with a small incision and faster dry-eye recovery; LASIK is more flexible for hyperopia and high astigmatism. How often are anti-VEGF injections needed for wet AMD?
Standard protocols use ranibizumab, aflibercept, brolucizumab or faricimab — 3 monthly loading injections, then treat-and-extend with intervals of 4-16 weeks based on OCT and fluid status. Most patients need 5-8 injections in year one, decreasing to 3-6 in year two. Faricimab and high-dose aflibercept (8 mg) extend intervals further. Memorial and Liv retina services use OCT-A and home Amsler monitoring to optimize re-treatment timing.
Should I treat glaucoma with drops, SLT laser or surgery?
First-line per EGS/AAO guidelines is either prostaglandin analog eye drops (latanoprost, bimatoprost) or selective laser trabeculoplasty (SLT) — both lower IOP by 20-30%. SLT is now offered as first-line based on the LiGHT trial, with 75% drop-free at 3 years. Surgery (trabeculectomy, XEN gel stent, Preserflo, MIGS) is reserved for progression despite maximal medical/laser therapy. Memorial and Akdeniz offer the full glaucoma armamentarium.
Where do donor corneas come from and how long is the wait?
In Turkey, donor corneas come through licensed eye banks compliant with Ministry of Health regulations, sourced from screened deceased donors with serology and slit-lamp evaluation. Memorial, Liv and Akdeniz typically schedule keratoplasty (DSAEK, DMEK or penetrating keratoplasty) within 2-6 weeks. DMEK is preferred for endothelial disease with 90-95% one-year graft survival and faster visual recovery.
What presbyopia treatments are available after age 50?
Options include multifocal/trifocal IOL implantation (RLE — refractive lens exchange), corneal multifocal LASIK (PRESBYOND laser blended vision), monovision LASIK, and pinhole corneal inlays. For patients over 50-55 with early lens changes, RLE with a trifocal IOL is often the most durable solution because it also eliminates future cataract surgery.
