What is the cure rate for hepatitis C with modern treatment in Turkey?
Direct-acting antivirals (DAAs) such as sofosbuvir/velpatasvir or glecaprevir/pibrentasvir achieve SVR12 (sustained virologic response at 12 weeks = cure) rates above 95% across all HCV genotypes. At Memorial, Liv and Akdeniz a standard 8-12 week oral course costs roughly $3,500-5,500 all-in, compared to $20,000-60,000 in EU clinics. Cirrhosis, prior treatment failure and HIV co-infection are managed without losing efficacy.
How is the duration of my HCV treatment decided?
Duration depends on three factors: genotype (1-6), fibrosis stage (FibroScan or APRI/FIB-4), and prior treatment history. Treatment-naive non-cirrhotic patients on pangenotypic regimens usually need only 8 weeks. Compensated cirrhosis or previous DAA failure extends therapy to 12-16 weeks, sometimes with ribavirin. Our hepatologists confirm the plan after baseline HCV-RNA, genotype, and liver elastography.
When should hepatitis B treatment actually be started?
Per EASL/AASLD guidelines, antiviral therapy (entecavir or tenofovir/TAF) is indicated when HBV-DNA exceeds 2,000 IU/mL with persistently elevated ALT, or at any viral load if there is significant fibrosis (F2+), cirrhosis, extrahepatic manifestations, or a family history of HCC. Inactive carriers are monitored every 6 months with ALT, HBV-DNA and ultrasound rather than treated immediately.
I had a needle-stick or risky exposure - is HIV post-exposure prophylaxis available?
Yes. HIV PEP must start within 72 hours of exposure, ideally within 2 hours. The standard regimen is tenofovir/emtricitabine plus dolutegravir or raltegravir for 28 days. Our infectious-diseases team performs baseline HIV, HBV, HCV and syphilis testing, prescribes PEP the same day, and arranges 4-6 week and 3-month follow-up serology. What workup is done for recurrent urinary tract infections?
After three or more culture-proven UTIs in 12 months we run urine culture with antibiogram, renal/bladder ultrasound, post-void residual measurement, and uroflowmetry. Women receive a vaginal exam; men receive a prostate evaluation. Cystoscopy and CT urography are added if hematuria, atypical pathogens, or upper-tract symptoms appear. Management includes targeted antibiotics, prophylaxis, vaginal estrogen where indicated, and behavioral measures.
How is Lyme disease diagnosed in Turkey?
Diagnosis follows a two-tier serology: ELISA for anti-Borrelia IgM/IgG, confirmed by Western blot if positive or equivocal. PCR of synovial fluid or CSF is used for Lyme arthritis or neuroborreliosis. Erythema migrans rash is diagnosed clinically - no testing needed. Treatment is doxycycline 100 mg twice daily for 14-21 days, or ceftriaxone IV for neurologic or cardiac involvement. Tick-borne co-infections are screened separately.
