Frequently asked about Radiology & Interventional Radiology

When do I actually need a 3T MRI instead of a 1.5T?
3T MRI doubles the signal-to-noise ratio, giving sharper images and shorter scan times. It is preferred for brain (MS plaques, tumors, epilepsy), pituitary, inner ear, prostate multiparametric MRI, musculoskeletal cartilage and small joints, and MR angiography. 1.5T is still equivalent or better for patients with implants, claustrophobia, very large body habitus, or routine spine and abdominal imaging. Memorial, Liv, Akdeniz and Memorial Antalya all run both.
PET-CT versus contrast CT - which one should I have for cancer staging?
For most solid tumors that are FDG-avid (lung, lymphoma, head-neck, esophagus, colorectal, melanoma, breast), PET-CT outperforms contrast CT in detecting nodal and distant metastases and changes management in 15-30% of cases. Contrast CT remains first-line for primary local staging, vascular planning and follow-up of established lesions. For prostate cancer we use PSMA-PET; for neuroendocrine tumors, Ga-68 DOTATATE PET-CT.
What types of image-guided biopsy do you perform?
Our interventional radiologists perform ultrasound-guided biopsies for thyroid, breast, liver, kidney and superficial lymph nodes, and CT-guided biopsies for lung, retroperitoneum, bone and deep abdominal lesions. MRI-guided biopsies are used for prostate fusion sampling. Core-needle and fine-needle aspiration are tailored to tumor type. Complication rates - pneumothorax, bleeding - stay under 2-5% with same-day discharge in most cases.
How often should I have a mammogram - and at what age?
Average-risk women: annual digital mammography from age 40 to 74, or biennial from 50 depending on guideline used (ACR/ACS/EUSOBI). Dense breasts (BI-RADS C/D) add supplemental ultrasound or abbreviated breast MRI. High-risk women (BRCA1/2 carriers, prior chest radiation, strong family history) start contrast breast MRI at age 25-30 alternating with mammography from 30. Tomosynthesis (3D mammo) is standard at Memorial and Liv.
Is TACE available for hepatocellular carcinoma, and who qualifies?
Yes - transarterial chemoembolization (TACE) is the standard locoregional therapy for intermediate-stage HCC (BCLC B): multinodular disease, preserved liver function (Child-Pugh A or early B), no vascular invasion, no extrahepatic spread. Drug-eluting bead TACE (DEB-TACE) gives more consistent dosing than conventional TACE. Our interventional radiology team also offers TARE (Y-90 radioembolization) and ablation (RFA, microwave) for selected lesions under 3 cm.
Can your radiologists give a second opinion on imaging done abroad?
Yes. Upload the original DICOM files (not screenshots) via our secure portal; we re-read them on diagnostic-grade monitors with the same PACS software the on-site teams use. A written second-opinion report in English plus your language is delivered in 48-72 hours, often with reconstructions or measurements the original report missed. Cost is typically $80-250 per study depending on modality and complexity.
I had an allergic reaction to CT contrast - what alternatives do I have?
True iodinated contrast allergy is rare; most reactions are mild. For confirmed prior reactions we offer (1) non-contrast CT with optimized protocols, (2) MRI with gadolinium - usually well tolerated even by iodine-reactive patients, (3) ultrasound or contrast-enhanced ultrasound with microbubbles, or (4) premedication with corticosteroids and antihistamines 12-13 hours before iodinated contrast in selected cases. eGFR is checked before any contrast.
