Functional rhinoplasty — what is included?
Septoplasty + turbinate reduction + cosmetic nasal reshaping in one operation. Recovery 7–10 days, splint removed at day 7, swelling resolves over 3–6 months.
Cochlear implant — bilateral or unilateral?
Bilateral is standard for children with bilateral profound loss. For adults the decision depends on residual hearing, age and budget — made jointly with the audiologist.
TORS for tonsil/tongue cancer — when is it indicated?
Early-stage oropharyngeal cancer (T1–T2). Avoids open neck incision, faster recovery. Multidisciplinary team confirms staging first.
Vertigo — do I need surgery?
About 90% of vertigo cases are managed with positioning manoeuvres (BPPV) or medication (Ménière). Surgery is reserved for endolymphatic sac decompression in refractory cases.
What is endoscopic sinus surgery (FESS) with image guidance, and do I need biologics?
FESS with CT-based navigation is offered for chronic rhinosinusitis failing 12 weeks of intranasal steroids, saline irrigation, and a steroid trial. Balloon sinuplasty is an option for isolated frontal or maxillary disease, while severe CRS with nasal polyps (CRSwNP) and eosinophilia is treated with dupilumab, omalizumab or mepolizumab in parallel. Day-case surgery, 95% symptom improvement at 1 year, follow-up debridement at weeks 1, 2 and 6.
Which sleep apnea surgery is best if I cannot tolerate CPAP?
Drug-induced sleep endoscopy (DISE) maps the obstruction site before any operation. Options include modified UPPP for palatal collapse, hypoglossal nerve stimulation (Inspire, FDA-approved) for BMI <32 and AHI 15-65 with no complete concentric collapse, and maxillomandibular advancement for severe craniofacial cases. Positional therapy devices are offered when supine-only OSA is documented; AHI reduction of 50-80% is typical.
How is the recurrent laryngeal nerve preserved during thyroid surgery?
Intraoperative neuromonitoring (IONM) with NIM electrodes is used routinely, reducing permanent vocal cord paralysis to under 1% in high-volume Turkish centers. Pre- and postoperative laryngoscopy documents cord mobility, and the parathyroids are mapped with near-infrared autofluorescence. Voice and swallowing therapy starts on day 2 if needed, and most patients return to normal speech within 2-4 weeks.
