Frequently asked about Gastroenterology

Can hepatitis C really be cured?
Yes — SVR12 (sustained virological response at 12 weeks) exceeds 95% with modern DAA regimens. 8 weeks for treatment-naïve without cirrhosis, 12 weeks otherwise. We monitor virological response after treatment.
IBD: when do I need biologics?
Moderate-to-severe disease, steroid-dependent or steroid-refractory, or extensive disease at diagnosis. Anti-TNF (infliximab/adalimumab) is first-line; vedolizumab if extra-intestinal manifestations.
Early stomach cancer — surgery or ESD?
If T1a (mucosal), well-differentiated, <2 cm and without ulceration — ESD is curative and avoids gastrectomy. Multidisciplinary team confirms staging.
Liver transplant — when is it considered?
MELD ≥15, decompensation, or HCC within Milan criteria. We refer to our hepatobiliary and transplant surgery team — see Organ transplantation.
Am I a candidate for endoscopic submucosal dissection (ESD) for early colorectal cancer?
ESD is offered for well/moderately differentiated T1a lesions under 30 mm without lymphovascular invasion, confirmed by magnifying NBI and EUS. En-bloc R0 resection rates in our JCI-accredited endoscopy units exceed 92%, with 5-year disease-free survival above 95%. Follow-up colonoscopy at 3, 6 and 12 months is standard, then annually.
Should I have capsule endoscopy or colonoscopy for suspected IBD or obscure GI bleeding?
Colonoscopy with ileoscopy and biopsies remains first-line for diagnosing Crohn's or ulcerative colitis. Capsule endoscopy (PillCam SB3) is added when small-bowel disease or obscure bleeding is suspected after negative upper/lower scopes, with diagnostic yield around 60-70%. A patency capsule is used first if stricture risk exists, and findings are correlated by an MDT within 7-10 days.
How is NAFLD/MASLD diagnosed and treated in 2026?
Diagnosis combines liver enzymes, FIB-4, and FibroScan (CAP for steatosis, kPa for fibrosis); biopsy is reserved for indeterminate F2-F3 cases. First-line treatment is 7-10% weight loss, GLP-1 agonists if BMI >30 or T2D, and resmetirom (Rezdiffra, FDA-approved March 2024) for biopsy-proven F2-F3 MASH. Patients are reassessed at 3 and 6 months by a hepatology MDT.
